ACE Inhibitors:When-and For Which Patients?

چکیده

Q:Should I avoid angiotensin-converting enzyme (ACE) inhibitors in my patients with progressive renal insufficiency? A:This issue is extremely bothersome to clinicians. ACE inhibitors represent a dual-edged sword with respect to their impact on renal function. On one hand, major clinical trials have clearly demonstrated their benefit in patients with congestive heart failure (CHF) and in patients with diabetic and nondiabetic renal disease. In multiple large clinical trials, ACE inhibitors reduce the risk of hospitalizations and death in patients with severe systolic dysfunction. In patients with proteinuric renal disease, ACE inhibitors significantly reduce proteinuria and slow the rate of progressive loss in renal function. Multiple benefits have been observed in older patients with cardiovascular disease, with or without hypertensionand with and without renal disease.1 These data, taken together, suggest that a significant segment of the population may benefit from this class of agents.

The effect of progressive aerobic continuous training on angiotensin-1, angiotensin-2 and angiotensin-converting enzyme type 2 in patients with heart failure

Background: Chronic hypertension causes structural and functional changes in the heart, ultimately leading to heart failure (HF), which further increases mortality and morbidit. HF is a complex clinical syndrome caused by various structural or functional abnormalities of the heart that impair the filling capacity of the ventricles. The findings of various trials have shown the association betwe...

مقایسه فعالیت ACE سرم و NAG سرم و ادرار در بیماران دریافت‌کننده ال- کارنیتین و جنتامایسین

Background and Aim: Nephrotoxicity is the most important and the most frequent adverse effect of Aminoglycosides, which restricts its usage despite its availability and effectiveness. It seems that   L-carnitine with an inhibitory effect on free radical production and some antioxidant effect, may have a supportive role against the potential renal damage due to ischemia. The recent study was d...

Study of the correlation between ACE gene polymorphism and coronary artery disease

Introduction: Angiotensin converting enzyme (ACE) is an exopeptidase that converts Angiotensin I to Angiotensin II. Angiotensin II is a potent vasoconstrictor and releases aldosterone, and have a critical role in hypertension. In this study, ACE insertion / deletion (I/D) polymorphism and ACE activity was determined in patients with coronary artery disease (CAD) and normal subjects. The corr...

ACE I/D and MMP-7 A-181G variants and the risk of end stage renal disease

The variants of angiotensin converting enzyme (ACE) and matrix metalloproteinases (MMPs) genes might be involved in the pathogenesis of end stage renal disease (ESRD) and hypertension. We studied the ACE insertion/deletion (I/D) and MMP-7 A-181G variants in 99 unrelated ESRD patients and 117 individuals without renal complications from Western Iran with Kurdish ethnic background. The frequency ...

Angiotensin Converting Enzyme Gene Polymorphism in Iranian Patients with Type 2 Diabetes

Background: Angiotensin I converting enzyme (ACE) is a Zinc metalloproteinase, converts Ang-I to Ang- II, a pro-inflammatory agent which may contribute to pathophysiology of some diseases like type 2 diabetes. Objective: To investigate the relationship between ACE I/D polymorphism and type 2 diabetes in 261 Iranian casecontrol pairs. Methods: 170 patients (85 type 2 diabetics with nephropathy a...
